2) If you will need to use the logo for any fundraising advertisement, items you are selling, etc. please complete this form as well and submit to the Director of Fundraising and Volunteers.

3) Also complete the logo permission form if using the logo and submit to the Director of Fundraising and Volunteers as well.

4) Once submitted and approved, the Director of Fundraising and Volunteers will obtain the fee required for the lottery license with the City of Woodstock. The Director of Fundraising and Volunteers will then submit it to the city and once approved, then you will receive approval and can purchase the prize(s) and print the tickets.
5) A reconciliation form MUST be completed after raffles. Please complete and send to the Director of Fundraising and Volunteers as well as the Treasurer.

6) After the raffle is complete, the money raised is submitted to the Woodstock Minor Hockey Association office and the raffle fee is taken off of the funds raised.
