ALL VOLUNTEERS MUST review and select an empty time slot from the Event's Volunteer Schedule posted on the WMHA Website for each event. IF POSSIBLE - please select a time slot without a volunteer already assigned (if a two person per time slot event is offered) - we are trying to assign at least one volunteer for each time before we fill the second spot. Your first choice is your Primary choice and the second a back up if your primary time has been taken. A WMHA committee member will contact you to confirm your request.

Volunteer's Signature

Must be signed before the time of the volunteer event

1. All volunteering dates and times will be posted on the WMHA Website for each event.
2. Select a primary date and time you wish to volunteer for and a secondary date and time. The secondary date and time will be used if the primary date and time has been filled.
3. A WMHA committee member will contact you if your primary or secondary date and time have been accepted. The filled time slots will also be posted on the WMHA Website for that event.
If both these time slots have been filled, the coordinator will contact you for an alternate date and time.
4. This form, once completed and BEFORE you submit it, must be printed. It is a two page document, try to print it on both sides of the paper.
5. The Director of Fundraising will contact you within a couple days before the event to follow up with your time.
6. All volunteers must be at the event at least 15 minutes before the volunteer time starts.
7. All teenage volunteers must be accompanied by an adult.
8. Once your volunteering time has been completed, return this form to the WMHA Office for approval. Only when the Volunteering Form has be approved and signed by a WMHA representative, then the WMHA Hockey Bond will be issued.
9. For all High School Student Volunteers; these hours can be used toward your high school volunteer credit requirements. Reminder that all teen volunteers must be accompanied by an adult.
