Vaccine Receipts Collection

Online Vaccine Receipt collection for players, bench staff and on-ice helpers.

This is mandatory for players and staff/volunteers to be able to be at ice times. The deadline is September 22nd, so please submit your receipts as soon as possible. Second deadline is October 17th with the second dose as the October 31st date includes the 14 day period after the second dose as required to complete series and be fully vaccinated.
